This is a notice to inform you that Dr. Jackly Juprasert will be leaving our practice, effective October 31, 2026.
Your care remains our priority, and we understand how important consistency in your medical care is. We want to assure you that your treatment and overall healthcare will continue without interruption.
Our practice is home to a dedicated team of board-certified physicians and advanced practice providers who are already familiar with our clinical protocols and ready to manage your care seamlessly. You are welcome to continue your treatment with any of our other exceptional providers, including myself.
If you have an upcoming appointment scheduled with Dr. Juprasert, our front office team will contact you shortly to reschedule your visit with one of our other providers. You do not need to take any action to keep your records at our facility. Because our practice remains the custodian of your medical records, your charts, imaging, and lab history will stay securely in our system if you choose to continue seeing another provider within our clinic.
If you instead choose to seek medical care outside of our practice, you have the right to request a copy of your records or have them transferred. Under Texas law and clinic policy, you may submit a completed, signed HIPAA Medical Records Release form to our Records Custodian by fax at 512-257-2426 or by email at DepartingMDrecords@Nuself.com. Please allow up to 15 business days for processing, and note that standard administrative copy fees will apply as permitted under Texas Medical Board rules.
